There are 2 reasons for an assessment to be closed.
1. For some reason, the consumer is no longer with QSI, but the assessment was left open. In this case, an administrator must close the assessment. You can reach an administrator by calling the Help Desk at 850-488-4357
2. The assessment is completed. If you do not select submit in each area of the assessment, it will not close properly. If your assessment is completed, but will not close, please start at the very beginning, and simply click submit in each section without making changes. Once you have done this, try again to close, and it should work. If you are still having trouble at this point, please contact the Help Desk for help from an Administrator.
